Please bring the following to your appointment:

• your current eyeglasses

• your current medical and vision insurance cards

• a list of your medications

• If you wear contact lenses, the prescription of the box(es)

• a good pair of sunglasses as your eyes will be dilated

Our staff is required to wear a face mask, as we are considered a medical clinic. We highly recommend that you also wear a face mask.
